Nice post! This happens to me when I reconcile unknown accounts from imported transactions sometimes. For example, an incorrectly capitalized letter results in two accounts. The numbers still add up correctly, but the account balances are wrong. I use the --strict option in ledger to make sure I have no unknown accounts when I'm finished.
